Understanding Treatment-Resistant Depression
The term sounds like a verdict. Clinically, it's just a category that unlocks a different set of tools.
What the term actually means
Inadequate response to two or more antidepressant trials at adequate dose and duration. That's it. It says nothing about your character, your effort, or your prognosis.
What's often missed
Before escalating, a thorough workup should rule out undertreated trauma, undiagnosed ADHD, bipolar spectrum illness, sleep apnea, thyroid dysfunction, chronic pain, and ongoing alcohol use. Any one of these can make a perfectly good antidepressant look like a failure.
What comes next
Evidence-based options include TMS, which is FDA-cleared for exactly this situation; augmentation strategies managed by your prescriber; and structured psychotherapy, which is frequently underused in patients who have been treated purely pharmacologically for years.
Why hope is reasonable here
Response rates for TMS after two failed medications generally exceed what a third medication would deliver. Running out of pills is not the same as running out of options.
